Insurance Information
Insurance and Fees: Payment is due at the time of service. As a courtesy, we complete reimbursement forms and send them to your insurance company for you.
HMO/PPO: We participate in more than 25 HMO/PPO's in the state. That number is constantly growing and we would gladly enlist to become a member of your plan. Some require co-payment and/or deductibles. Please call our staff for details.
Medicare: To better service our senior patients, we participate in the Medicare program and accept assignments. Workers compensation cases are welcome.
